“Patient Outcomes should be a priority in the nursing care that nurses deliver to patients. During a nurse’s every day practice they have a responsibility to identify risk factors and implement strategies to prevent adverse outcomes from occurring to patients. Each clinical setting has its unique issues that may be more prevalent than in other areas.”
This is an INDIVIDUAL assessment task. Students must research, evaluate and discuss a current issue impacting on patient outcomes and analyse what current or potential new strategies might be used to prevent these adverse outcomes from occurring to patients in the future.
Here are some examples to consider:
 As a student nurse you have observed within your current clinical placement that there is a tendency for the nursing staff to often use chemical or physical restraints in order to control patient aggression / agitation rather than using other diversional techniques or other strategies available to them. Could this clinical issue have an effect on patient outcomes?
 As a student nurse you have observed that staff members on a certain ward often are not using basic infection control measures when emptying bedpans / urine bottles. Then you observe that the staff members are then manually cleaning out the bedpans prior to inserting them into the bedpan washer. Review the current hospital policies concerning this infection control issue and discuss the potential adverse patient outcomes associated with this practice?
 Current practice for safe transferring of patients from the Emergency Department to other departments can vary within an Acute Hospital. For example: An Advance Life Support (ALS) Registered Nurse has to transfer patients from the Emergency Department to any Critical Care Area whereas, if a patient is transferred to the Medical Ward – often a Registered Nurse (without ALS training) is involved in the transfer. However, patients being transferred to the Medical Ward can be at higher risk of deterioration. What potential effect might this have on patient outcomes?
 As a student nurse you observe that a full patient assessment (head to toe examination) is not being conducted by the Registered Nurse admitting a patient. Could this clinical issue have an effect on patient outcomes?
 As a student nurse you observe other ward staff routinely looking up and print out their own pathology and medical diagnosis test results or their friend’s results etc. Could this clinical issue have an effect on patient outcomes?
Your Nurse Unit Manager has requested that you compile a clinical discussion paper that will form a basis for a future presentation to be given to your nursing colleagues. Your target audience for this future verbal presentation will be: Clinical Nurse Educator, Registered & Enrolled Nurses and/or Student Nurse Peers who are working within the clinical setting that you are currently placed or within your most recent clinical placement setting.
